2.  ALL OTHERS — I need a W8BEN form from you.  It can be found here:  W8BEN Form

I need either a W9 or a W8BEN form from everyone even if we are not paying for your airline tickets.

You can download these forms, fill them out, scan them and e-mail them to me.  Or you can download, fill in, sign, and fax them to me.  Or you can download, fill in, sign, and mail to me at Department of Communication, Box 7347, Wake Forest University, Winston-Salem, NC 27109.
